Instructions for Online Registration
Please follow the directions closely. It may help to read it entirely

- Create a folder on your computer desktop and label it, "Registration Forms".
- Download all of the forms below and save them into the new folder that you just created on your desktop. Please note that you can download either the Microsoft Word or PDF format to open and fill out, you will just need to download the forms of the program you are most familiar with. (PLEASE NOTE: The only way to fill out and upload the PDF forms online is to use Acrobat Pro. They can be opened with Adobe Reader and filled out, but you will not be able to save them and will only be able to print them out to turn them into the school).
- There should be a total of 4 forms for returning students and 5 forms for new students. PLEASE TAKE NOTE: There are 2 more additional forms that require you to actually fill out and sign, so you will not be able to submit these online. These two forms are the Consent to Treat form and the Media release form. The best way to have these 2 forms completed beforehand is to print them out, fill them in and return them to the school. - After each form is downloaded into your folder, you will open them one at a time with either Microsoft Word or Acrobat Pro/Adobe Reader, depending on which forms you downloaded. Fill in the each blank so that each highlighted area has been filled in. Please use "NA" if a highlighted area does not apply to you or your child. Also, it may help if you change the view so that it is easier to read.
- After the entire form is completed you will need to save it. Choose "save as" and select the same registration folder on your desktop to save it into that it was originally, with the same original name. You should receive an alert telling you that "a file with this name already exist. Do you want to replace it?" You DO want to replace it. If you have multiple children, please add the child's name at the end of the file name so we will know who the form belongs to. This will also prevent you overwriting a form that has already been filled in and saved.
- Once you have completed and re-saved each form, you will need to use the section below to upload and submit your forms. Unfortunately, at this time you are only able to submit one form at a time. Another option would be to attach each completed form into an email and send them all to "[email protected]". If you choose this option, please put "KAS Registration Forms and the year you are applying for" in the subject line.
- Each time that you submit a form you will need to enter your name, email address and attach each form from the registration folder on your computer.
- After you have submitted one form, you will need to click the link under the confirmation message you receive to submit another form.
